Medi-Cal Doula Benefit
California Department of Health Care Services (DHCS)
Can Medi-Cal pay for a doula, including after I have already given birth?
What this is
Doula services have been a statewide Medi-Cal preventive benefit since January 1, 2023, in both fee-for-service and managed care. A doula gives you nonmedical physical, emotional, and informational support through pregnancy, birth, and afterwards.
Why it matters after birth
You can start doula support after you have already given birth — this is not only a birth service. It also covers support through pregnancy loss, which is care that is often hard to find.
You may qualify if
You qualify if you are a Medi-Cal member who is pregnant or who was pregnant within the past year. For your initial set of services you can rely on DHCS's standing recommendation — you do not need to separately obtain a clinician's recommendation first. Postpartum-only use is allowed, and support for pregnancy loss, including miscarriage, stillbirth, and abortion, is included.
What you actually get
The initial set is 1 initial visit, up to 8 additional prenatal or postpartum visits in any combination, support for labor and delivery or for pregnancy loss, and up to 2 extended 3-hour postpartum visits. A second recommendation can authorize up to 9 additional postpartum visits.
How long it lasts
Available from pregnancy through 1 year after the pregnancy ends.
What it costs
Doula services are a covered Medi-Cal preventive benefit. Cost rules differ across Medi-Cal pathways, so confirm with your plan for your own coverage.
How to use it
If you are in fee-for-service Medi-Cal, use the DHCS Doula Directory to find a doula. If you are in managed care, call your plan's Member Services and ask for their enrolled doulas. For your first set of services you can rely on DHCS's standing recommendation rather than chasing a clinician's note.
What you'll need
Your Medi-Cal member and plan information. For the initial set of services, DHCS's standing recommendation applies.
Important limitations
The doula has to be enrolled and, in managed care, appropriately contracted — so the practical question is who is available in your county and your plan. DHCS reported 1,768 enrolled individual doulas as of September 11, 2026, but availability varies by county and plan. Doula support is nonmedical and does not replace your clinical or mental-health care.
How it works with other benefits
It sits alongside your maternity care, behavioral-health care, and CPSP support inside your 12-month postpartum coverage.
Finding a provider or service
Fee-for-service members use the DHCS Doula Directory, which includes county, specialty, language, and ethnicity and contact information. Managed-care members call Member Services. Confirm the doula can bill your specific coverage.
Language & cultural access
The DHCS Doula Directory includes language and ethnicity information, so you can look for a doula who matches your language and community. Ask your plan for interpretation if you need it.
If you do not qualify
If you are not a Medi-Cal member, ask community doula organizations about private-pay, grant-funded, or sliding-scale support, and confirm cost before services begin.
Access in practice
Being covered and being able to use it are different things. This is what the sources say about actually reaching this support.
- Provider availability
- 1,768 enrolled individual doulas as of September 11, 2026; availability varies by county and plan.
- Provider rules
- The doula must be enrolled, and appropriately contracted where managed care applies.
- Referral or prior authorization
- DHCS's standing recommendation covers the initial set of services; a second recommendation can authorize up to 9 additional postpartum visits.
In plain terms
What this means for you
You can have a doula paid for even if you have already given birth, and even after a loss — and for your first set of visits you do not need to go back to a clinician for a recommendation. The real work is finding one who is enrolled and available in your county, and the directory lets you search by language and community.
Your next step
What to do next
If you are in managed care, call your plan's Member Services today and ask for their enrolled doulas in your county. If you are in fee-for-service, search the DHCS Doula Directory by county and language. Say clearly how far postpartum you are so they book the right visits.
