Contacting our office to schedule an appointment. There is paperwork to fill out for basic informaion (Name, Address, Child information, etc). To receive a crib/pack n play. The child needs to be under 1 year old and under 30 lbs for the pack n play. We will evaluate the need on an individual basis.
