Eligibility
Families must meet all of the following criteria to be eligible for the LFS SleepSafe Response Program: 1. Child Age Requirement • The child in need of the crib must be between birth and 5 years old. 2. Lack of Safe Sleeping Arrangement • The child is currently sleeping on the floor, couch, in an unsafe shared space, or has no designated bed. 3. Residency Requirement • The family must reside in Philadelphia. 4. Income Eligibility • Household income must be at or below 200% of the Federal Poverty Level (income documentation may be required). 5. The parent or guardian must enroll in one of the following LFS programs: • MOMcares Prenatal Outreach Program (for pregnant or postpartum parents) • Tiny Essentials Relief Program (for parents/caregivers of children under 4) 6. Safe Sleep Education Completion (Required by Cribs for Kids®) • Families must complete safe sleep education in one of the following ways: • Participate in a live education session (virtual or in-person) through LFS OR • Complete the Cribs for Kids® Safe Sleep Ambassador Online Training, including a post-test score of 90% or higher → Completion certificate or screenshot must be submitted to LFS. 7. Consent Form • The parent/guardian must sign the LFS SleepSafe Program consent form, agreeing to use the bed as intended and follow safe sleep practices. 8. Sliding Fee Contribution (if applicable) • Based on income, families may be asked to contribute up to $100 toward the cost of the crib or bed. → No family will be turned away for inability to pay.
