Eligibility
Screened by the CHW through the PRN program. Under 6 months of age, fill out and sign the agreement form, agree to receive education on safe sleep guidelines, no other safe sleep environment, attest that they do not have the means to acquire a safe sleep surface and they will not accept a portable crib from any other program or resource. On the form it also states that they agree to let the CHW survey them on safe sleep practices until the child is one and that the portable crib is for personal use only.
