Eligibility
Call the Nursing Division of the Health Department at 330-627-4866 ext 1530, walk ins welcomed. Families are eligible to receive a Cribs for Kids® crib through CCGHD if they meet any of the following criteria: Pregnant women in their third trimester or caregivers of an infant under 1 year of age. Medicaid-eligible families or families with demonstrated financial need. Families residing in Carroll County. Willingness to participate in safe sleep education provided by a trained Public Health Professional or Community Health Worker. Agreement to use the crib and other items according to safe sleep guidelines.
